Each participant in the Big Group Games (children and youth) and each participant in the afternoon activities (Giant Swing & Archery) will need to complete the following Medical and Consent form for VERTEC.

To save time on the day, please complete these forms by 30th December 2022 and return them to Jill.

Printed copies will be available from the SCPC office if you require.
